Thread Lift vs Dermal Filler: Which One is Right for Your Face Shape?
Two of the most common questions we hear at Vivardi Clinics: “Should I get a thread lift or filler?” and “Will filler make my face look fat?” Both come from the same place – a patient who wants to look refreshed, not overdone, and has no idea which treatment actually addresses their concern.
The confusion is understandable. Both are non-surgical, both involve injections, and both are promoted as “anti-aging” treatments. But they work completely differently, solve different problems, and are suited to different face shapes and age groups. Getting the wrong one will at best give you mediocre results, and at worst make your concern worse.
This guide is written by Dr. Dinesh Kumar, Medical Director at Vivardi Clinics Rawang, to help patients make an informed decision before their consultation – not to replace one.
The Core Difference: Lift vs Volume
Before comparing the two treatments, you need to understand what actually happens to your face as you age. Two things occur simultaneously – and they require different solutions.
First, your skin loses support and begins to sag. The SMAS layer (the structural layer beneath your skin) weakens, and gravity does its work. Jowls form. The jawline softens. Nasolabial folds deepen. This is a mechanical problem – the architecture of the face is drooping.
Second, you lose volume. Fat pads in the cheeks, temples and under-eye area shrink. Bone density reduces slightly. The face begins to look hollow, tired or gaunt even when the skin itself is in decent condition.
- Sagging skin along the jawline and cheeks
- Jowl formation and loss of facial contour
- Drooping of the mid-face and brow
- Loose skin along the neck
- Loss of the V-shape facial profile
- Hollow cheeks and sunken temples
- Thin or asymmetrical lips
- Under-eye hollows (tear troughs)
- Flat or undefined chin and jawline
- Deep nasolabial folds from volume loss
Adding filler to a sagging face is like inflating a deflated balloon – it makes the sagging area look bigger, not lifted. The right treatment must match the right problem.
Dr. Dinesh Kumar, Medical Director – Vivardi ClinicsWhich Treatment Suits Your Face Shape?
Face shape is one of the most reliable guides to which treatment will deliver the most natural-looking result. Here is how we typically approach it in clinic.
Wide cheekbones with narrow chin and forehead. Thread lift refines the jawline without adding width to an already wide mid-face.
Strong jawline with equal width throughout. Chin filler and cheek filler add softness and vertical elongation.
Naturally balanced proportions. Thread lift maintains contour with age while filler restores volume loss in cheeks and temples.
Wide forehead, narrow chin. Thread lift lifts the mid-face and defines the jawline without widening the lower face.
Full cheeks with soft jawline. Chin and jaw filler creates vertical elongation and more defined angles.
Long and narrow face. Cheek filler adds width while thread lift prevents further sagging along the sides of the face.
Face shape is one factor – age, degree of skin laxity, bone structure and existing volume all influence the final recommendation. A consultation with a doctor is always required before treatment.
Thread Lift: What You Need to Know
A thread lift uses fine dissolvable threads – typically made from PDO (polydioxanone) or PLLA (poly-L-lactic acid) – inserted under the skin with a fine needle. The threads have tiny barbs or cones that anchor to the tissue and physically lift the skin when the thread is tensioned.
How it works mechanically
Once inserted, the threads do two things. First, they provide immediate mechanical lifting by physically repositioning the soft tissue. Second, over the following weeks, they stimulate collagen production around the thread, which provides longer-term structural support even after the thread itself has dissolved.
What thread lift is good for
Thread lift is most effective for patients in their mid-30s to early 50s with mild to moderate skin laxity. Patients with very loose or significantly aged skin may not get enough lift from threads alone and might be better served by a surgical facelift consultation.
Downtime and what to expect
Most patients experience mild swelling and bruising for 3 to 7 days. Some temporary dimpling of the skin at insertion points is normal and resolves within 1 to 2 weeks. Results are visible immediately but improve over 4 to 6 weeks as collagen builds. Results typically last 12 to 24 months.
Dermal Filler: What You Need to Know
Dermal fillers are gel-like substances injected beneath the skin to restore lost volume, smooth lines or enhance facial contours. The most common type used in aesthetic clinics is hyaluronic acid (HA) filler – a substance naturally found in the body that attracts and retains water.
How it works
HA filler integrates with the surrounding tissue and physically adds volume to the area injected. Results are immediate. Unlike thread lift, filler does not lift – it fills. Adding filler to a sagging area without first addressing the structural laxity can sometimes worsen the appearance by adding bulk without lift.
Different fillers for different areas
Not all fillers are the same. Thicker, more cohesive fillers are used in structural areas like the chin, jawline and cheeks. Softer, more fluid fillers are used in delicate areas like the lips and tear troughs. Using the wrong filler product in the wrong area is one of the most common causes of poor results.
Longevity and reversibility
HA fillers last 6 to 18 months depending on the product and area. One significant advantage is that HA filler is reversible – an enzyme called hyaluronidase can dissolve it if you are unhappy with the result or if a complication occurs. This makes it a safer choice for first-time patients.

Who is Each Treatment Best For?
- Mild to moderate skin sagging along jawline or cheeks
- Patients in their mid-30s to early 50s
- Those who want to avoid surgery but need actual lift
- Diamond or heart face shapes needing jawline definition
- Patients who have already tried filler but feel it didn’t address their sagging
- Those wanting collagen stimulation as a long-term skin investment
- Hollow cheeks, sunken temples or under-eye hollows
- Patients in their late 20s to 40s with volume loss but minimal sagging
- Round or square face shapes needing proportion adjustment
- Lip augmentation or chin enhancement
- First-time aesthetic patients who want a reversible option
- Those with asymmetrical facial features needing balancing
Can You Combine Both Treatments?
Yes – and for many patients, this is actually the most effective approach. When sagging and volume loss occur together (which is common from the early 40s onwards), treating only one without the other often leaves patients feeling like something is still “off” after treatment.
The sequence matters. Typically, threads are placed first to establish the structural lift, then filler is added conservatively to restore volume in areas that remain hollow after lifting. Doing it in reverse – adding heavy filler first then threading – can make the result look unnatural.
Combining threads and filler should only be done by an experienced doctor who can assess whether the timing and sequence are appropriate for your specific anatomy. Never let this be done in the same session by two different practitioners.
Frequently Asked Questions
How to Choose: A Simple Framework
When patients sit down with us for a consultation, we walk through a simple set of questions to determine the right direction. You can ask yourself the same questions before your visit.
Ask yourself: Is my main concern sagging or hollowness?
Stand in front of a mirror and gently place your fingertips on your cheekbones and push the skin upward slightly. If this makes your face look significantly better, your main concern is sagging – thread lift is likely your primary solution. If the improvement is minimal and your face still looks tired or gaunt, the concern is more likely volume loss – filler is your starting point.
Ask yourself: How old is my skin?
If you are in your late 20s or early 30s with no visible sagging, filler alone is almost certainly sufficient and thread lift is premature. If you are in your 40s or beyond with clear jowling or mid-face descent, threads become the more relevant conversation.
Ask yourself: How much downtime can you afford?
Filler has minimal downtime – most patients are social-ready within 24 to 48 hours. Thread lift requires more recovery with 3 to 7 days of visible swelling and some restriction on facial movement and sleeping positions. If you have an event in two weeks, filler is the safer timing choice.
