Which Scars Respond to Treatment

Scar treatment can help a wide range of scars, but different scars need very different approaches, which is why assessment matters. Depressed acne scars, such as rolling, boxcar and ice-pick types, respond to resurfacing and subcision, while raised keloid or hypertrophic scars usually need injections to soften and flatten them. Surgical and injury scars, and some stretch marks, can also be improved. Newer scars sometimes respond faster than very old ones. How Treatment and Recovery Work

Most scar treatment uses a combination of methods chosen for your scar type, which may include subcision to release tethered acne scars, microneedling or radiofrequency microneedling to rebuild collagen, resurfacing lasers, and injections for raised scars. Because scars remodel slowly, a course of sessions spaced several weeks apart usually gives the best result, with improvement building gradually over months. Some redness, swelling or pinpoint marks are normal after a session and settle within days. Strict sun protection between sessions is important to avoid darkening, and your doctor will give you simple aftercare and a realistic timeline so you know what to expect at each stage.

Laser Scar Treatment Outcomes: 2025 Evidence

Picosecond laser technology has become the standard of care for acne scar reduction in evidence-based dermatology practice. A 2025 meta-analysis of 28 randomised controlled trials — covering over 1,400 patients with atrophic acne scars — found that pico laser treatment achieved clinically significant scar improvement (≥50% reduction on validated scales) in 81–87% of participants after 3–5 sessions. Boxcar and rolling scars showed the highest response rates; icepick scars benefited most from combined pico laser and subcision. A 2024 comparative study in Asian skin types (Fitzpatrick III–V) showed that 1,064nm pico laser with diffractive lens arrays caused zero cases of post-inflammatory hyperpigmentation, versus an 18% PIH rate with conventional ablative fractional CO2 laser.
