What is Premature Ejaculation ?
The ejection of sperm from the body known as ejaculation. Premature ejaculation has been explained in a variety ways. Premature ejaculation is a male sexual disorder in which ejaculation occurs with minimal stimulation, before or nearly before vaginal penetration, In general, PE is when ejaculation occurs earlier than a man or his partner prefers during intercourse . It happens with little control, often within one to two minutes of penetration or even before the intercourse begin. This can lead to frustration, anxiety and difficulties in sexual relationships.

Erectile dysfunction (ED) is not the same as PE. The failure to obtain and sustain an erection that makes for a pleasurable sexual experience is referred to as ED. PE, on the other hand, may occur in conjunction with ED.
How does ejaculation occur ?
- The central nervous system is in control of ejaculation. Signals are sent to the spinal cord and brain when men are sexually aroused. Upon reaching a certain level of excitement, the signals are transmitted from the brain back to the reproductive organs resulting in semen being ejected through the penis (ejaculation).
There are two phases in ejaculation :

- Emission Phase:
- This is the first phase, during which semen is moved from the testes, prostate, and seminal vesicles into the urethra.
- It is controlled by the sympathetic nervous system, and at this point, ejaculation becomes inevitable. The man may feel a sensation of building pressure or the “point of no return.”
- Expulsion Phase:
- This is the second phase, where the muscles around the base of the penis and in the pelvic area contract rhythmically, pushing semen out through the urethra.
- This process is also controlled by the nervous system and leads to the physical act of ejaculation.
Premature ejaculation occurs when these phases happen too quickly or without enough voluntary control.
Main Types of Premature Ejaculation :
- Lifelong (Primary) Premature Ejaculation:
- This type starts from a man’s first sexual experiences and persists throughout his life.
- It is usually linked to biological or genetic factors.
- Acquired (Secondary) Premature Ejaculation:
- This type develops after a period of normal sexual performance.
- It is often associated with psychological issues like stress, anxiety, or depression, or physical conditions like erectile dysfunction or prostatitis.
- Each type may require different treatment approaches based on the underlying causes.
CAUSES OF PREMATURE EJACULATION :
1 . PSYCHOLOGICAL CAUSES OF PREMATURE EJACULATION:
- Performance Anxiety: Fear of not satisfying a partner can lead to nervousness, causing a man to ejaculate prematurely. This anxiety makes it difficult to focus on controlling the timing of ejaculation.
- Stress and Relationship Problems: Emotional stress, either from work or relationship issues, can lead to reduced sexual control. Tension between partners or unresolved conflicts can also add to performance pressure.
2. BIOLOGICAL CAUSES OF PREMATURE EJACULATION :
- Hormonal and Neurotransmitter Imbalances: Abnormal levels of testosterone or low serotonin can affect the brain’s ability to delay ejaculation. These imbalances disrupt the normal sexual response cycle, leading to quicker ejaculation.
- Prostate and Erectile Dysfunction: Prostate issues like infections or inflammation can trigger PE. Additionally, men with erectile dysfunction may rush to ejaculate before losing their erection.
- Genetic Factors: Some men may be genetically predisposed to ejaculate more quickly.
TREATMENT OPTIONS FOR PREMATURE EJACULATION :
Treatment options for premature ejaculation (PE) can include a combination of behavioral techniques, therapy, medications, and lifestyle changes. The choice of treatment depends on the underlying cause, whether it is psychological, biological, or both.
1.BEHAVIORAL TECHNIQUE :
- STOP START METHOD :This involves stimulating the penis until near ejaculation, then stopping to allow the sensation to subside. Repeating this process helps gain control over ejaculation.
- SQUEEZE TECHNIQUE :In this method, the base of the penis is gently squeezed just before ejaculation to reduce arousal and delay the climax.
2. PSYCOLOGICAL THERAPY :
- Men Counselling : Psychological counseling is a method of working with the thoughts and emotions that can contribute to intimate relationship issues. The aim of this form of therapy is to figure out what’s causing the problems and come up with ideas that will help PE.

- Couples Therapy: This can help resolve relationship conflicts that may contribute to premature ejaculation.

- MEDICATIONS :
- Topical Anaesthetics: Creams or sprays (like lidocaine or prilocaine) can numb the penis and reduce sensitivity, delaying ejaculation.

- Oral Medications: Some antidepressants (like selective serotonin reuptake inhibitors, or SSRIs) and pain medications can delay ejaculation as a side effect.

- PDE5 Inhibitors: Medications like sildenafil (Viagra) used for erectile dysfunction can help men maintain erections and delay ejaculation.

4 .KEGEL THERAPY

Kegel machine EMF (Electromagnetic Field) therapy for premature ejaculation (PE) is a non-invasive treatment that uses electromagnetic energy to stimulate and strengthen the pelvic floor muscles. This therapy is designed to help men improve control over ejaculation by enhancing the strength and coordination of the muscles involved in sexual function, particularly those in the pelvic floor.Usually it is done 2-3 times per week for several sessions to obtain maximum long lasting effect.

Kegel machine EMF therapy is often offered in clinics or specialized treatment centers, and it can be part of a comprehensive approach to treating PE, along with other therapies like behavioral techniques or medications. However, it’s important to consult a healthcare professional to determine whether this treatment is appropriate based on individual needs.
